I write to you as a constituent. I write in memory of Adrian Moynihan. I write so that what happened to Adrian and his family can never happen again.

We all hope that when a loved one dies, they go peacefully. If someone dies tragically, their loved ones should be met with compassion as they try to find answers and peace.

It’s important, too, that justice is done. But that’s not what happened in Adrian’s case. Despite clear evidence that Adrian died because of human intervention, the inquest returned a verdict of death by misadventure. The truth of how Adrian died has never been uncovered.

Adrian’s family have no way to make a complaint about this verdict. They have found legal irregularities in the makeup of the inquest jury, but they have no way to question this either.

This must change now. Government must take action to reform the coroners from the ground up, as recommended by the Irish Council for Civil Liberties.

Crucially, the reform process must include a complaints mechanism. Otherwise, how can we ever hope to guarantee justice for people like Adrian?
